Concussion Awareness (Rowan’s Law):

Participant Over 18 Years of Age
I confirm that I will review any one of Ontario’s official Concussion Awareness Resources before registering for a program with Mayfair Clubs and will review the Concussion Code of Conduct that will be provided to me upon confirmation of registration.

Participant Under 18 Years of Age
As the parent/guardian of a participant under the age of 18 years, I confirm that I will review any one of Ontario’s official Concussion Awareness Resources before registering for a program with Mayfair Clubs and will review the Concussion Code of Conduct that will be provided to me upon confirmation of registration.

Cancellation Policy:

Mayfair Clubs requires a minimum of seven (7) days written notice from a registrant looking to cancel an event and/or activity. Cancellation requests received more than 7 days prior to the beginning of the program will be issued a credit for a future program. No credits will be issued for cancellation requests for any reason received less than 7 days prior to the beginning of the program. Please note: We do not offer refunds and we do not offer make-up classes/sessions/days.

Mayfair Clubs reserves the right to cancel any program on any given date and time for situations outside of Mayfair Clubs control. Should this occur, all registrants will receive a credit for a future program. If the program has begun, a credit will be issued for the unused portion of the program.

Assumption of Risks:

I am aware that by participating in a Mayfair Clubs’ activity involves many risks, dangers and hazards including, but not limited to: collision with other persons or objects; being struck by equipment; being hit by a ball; heart attack, or other similar life threatening conditions caused by physical exertion; the risk of personal injury including, but not limited to, strains, sprains, fractures, brain injury, spinal cord injury including paraplegia or quadriplegia, or death; loss of balance or control; slips, trips and falls; negligent first aid; failure to act safely or within one’s own ability; negligence of other persons; and, negligence on the part of the releasees. I understand that negligence includes failure on the part of the releasees to take reasonable steps to safeguard or protect the participants from the risks, dangers and hazards referred to above. I freely accept and fully assume all such risks, dangers and hazards and the possibility of personal injury, death, or property damage or loss resulting therefrom, on behalf of the participants.

I expressly agree that Mayfair Clubs, its officers, directors, employees, servants or agents, shall not be liable in any manner whatsoever for any damages of any nature or kind whatsoever arising from any injuries while partaking in, or after, this event; nor shall Mayfair Clubs, its officers, directors, employees, servants or agents, be held liable for any injury, loss or damage is caused by an act of negligence or omission of staff, management or any other person. I hereby grant Mayfair Clubs permission to use any photographs or video of the participant in publications, displays, presentations or other related use in any format (print, electronic, www site, or other media) for the purpose of promoting Mayfair Clubs events, activities and projects. I waive any copyright or other rights I may have in use of images.

Mayfair Clubs Membership Pricing Policy:

Exclusive Pricing for Existing and Active Members

Existing and active Mayfair Clubs members are entitled to exclusive pricing on our programs. When you go to check out your registration, you will be asked to identify the participant as a Member or Non Member. Once you select the option for Member, a discount will be applied to your purchase. The participant being enrolled into the activity MUST be an active Mayfair Clubs member in order to receive the discounted price. You will need to provide the participant’s Membership number when you complete your registration. Membership numbers can be found on your renewal letters.

Non-Member Registration and Payment

Non-Members can register for our Camps and Programs. However, if you register for a program or camp, and indicate that you are a member without being an active Mayfair Clubs member, you will be charged the price difference between the non-member fee and the amount paid for the member fee. Additionally, an administrative processing fee may be applied in some cases.

Pricing & Payment Adjustments

If our team contacts you via the email on file to inform you that your membership is not active or on file and you need to pay the difference to account for the non-member rate, applicable fees will be incurred.

Payment Deadline

You will have 3 business days from the date of notification to provide payment.

Late Payment Fees

If payment is not received within the 3-business day period, an additional $25 administrative processing fee will be applied, in addition to the price difference that will be charged.

Cancellation Policy

If the situation is not resolved within 10 business days, we reserve the right to cancel your registration and refund your registration fee.

Please ensure that you are providing the correct information regarding your Membership status when registering for any programs to avoid any inconvenience.

Late Pick-Up Policy (Kids Camps):

We understand that unforeseen circumstances may arise, causing delays in picking up your child from our summer camp. To ensure the safety and well-being of all campers and staff, we have implemented the following late pick-up policy:

Lakeshore:

Parkway:

West:

Splash & Laugh and Multi Sport:

Tennis:

Pickleball:

Late Fee: $1 per minute, charged in increments of one minute.

The purpose of this policy is to ensure the safety and well-being of all campers by maintaining appropriate supervision levels after regular camp hours. It also aims to discourage late pick-ups that may inconvenience camp staff and other families.

Exceptions:

Exceptions to the late pick-up policy may be made in cases of emergencies or unforeseen circumstances. Please contact the front desk as soon as possible if you anticipate being late due to such circumstances.
